Can a design application be restricted by the Examiner, even though design applications may contain only one claim?

Yes.  Even though a design application contains only one claim, more than one embodiment of a design can be protected by a single claim.  However, if the design application contains more than one patentably distinct design, the Examiner will issue a restriction.  See MPEP 1404.05.
